Volunteer Forms

Our number one priority is safety. All of our deans, faculty, jr. faculty and other volunteers need to complete the following forms prior to their arrival at camp. If you are age 21 or older and are not a resident of Indiana or Michigan, please contact the camp office to get the Child Protective Services form for your state.

For All Volunteers

NEW FORM! All volunteers/staff need to complete the Personnel Record Form. All previous forms have been combined into this one form. Please complete this questionnaire. If you are over the age of 21, please look below to also complete the Central Registry Clearance.

For Volunteers age 21 or Over

Indiana Residents

NEW PROCESS!!  All Indiana Residents age 21 or over need to complete their own Central Registry Clearance by clicking the link below and completing this form.  You will need to scan and email them to Kelly at kelly@michianacamp.org or fax them to 269-683-4402.  Soon after the office receives them, you will receive two emails from KidsTrak. Please follow the instructions included in those emails. Once that step is completed, your responsibility is done and the State of Indiana will send results to the camp.

Michigan Residents

NEW PROCESS!! Michigan has changed from paper forms to online only. You must create an account by clicking below and enter your own information. Please contact Kelly at the camp office if you have any questions.

The process is simple, free, and quick—most results are available within 5 business days.

To get started:

  1. Visit michigan.gov/ccbc-camps.

  2. Create a new account.

  3. Log in and submit a Central Registry clearance request.

Once your clearance is complete, you will have the option to:

  • Download the letter,

  • Print a copy; and/or

  • Share it electronically using a secure link.

Please note: The camp is required to keep a copy of your Central Registry clearance on file, so be sure to provide your results using one of the above options.
